This brief questionnaire will only take 5 minutes to complete, but it is a million dollars' worth of information for people just like you visiting this site. Feel free to do it totally anonymously. But, please do it.

As you have seen, the site is solely here to provide a detailed information service to alcoholism sufferers who are either taking Baclofen or considering taking Baclofen. This is unchartered territory, and it is important that we collate as much information as possible.

Soon, I hope to have a standard 'Visitors' Questionnaire' for you to complete in seconds. Pending that, I would be really grateful if you would take just a couple of minutes to let me know a few simple details about you, how you got here, your experiences, and such like. I do not expect an essay!

• On a scale of 1 (poor) to 5 (excellent) how would you assess your current Baclofen?
